Creating bookmarks and hyperlinks in Acrobat Pro 5

I can still use Acrobat Distiller as a printer to create PDF output, and use Acrobat Pro to edit the output. What I can’t do is automatically create an Acrobat file that has a hyperlinked Table of Contents (clicking on a page number take you to that page) or live URLs. Tags: Software.
