Some are medium sized and open, but still centered around a discipline. Discussion boards can be added to your website, leveraged in a Ning site, or you can use a google group or similar solution. Consider Ning, Basecamp, and Zoho. If you open up comments to all users, you are going to get spammers. Some are huge.
