Creating bookmarks and hyperlinks in Acrobat Pro 5

I’ve tried lots of free PDF creation software and haven’t found one that resolves these problems. To insert bookmarks (internal links) in an Acrobat Table of Contents. With the Link Properties dialog open, scroll to the page where the bookmark should be placed. Instead I’ve found some workarounds.
